The majority of our patients live in Assisted Living Facilities (ALFs), which offer a supportive and comfortable living environment. These communities provide a range of services, including meals and personal care such as dressing, bathing, medication reminders, and housekeeping.
Many facilities also offer 24-hour assistance, providing added peace of mind for residents and their loved ones.

Our Patients
At First Choice Community Medical Services, we provide care directly in the setting where our patients live—most often within assisted living and residential care communities.
To qualify for care under Medicare guidelines, patients are typically considered “homebound.” While the term may sound restrictive, it simply means that leaving their residence requires assistance or a considerable effort.
Many patients are still able to enjoy outings with support—like spending time with family or attending personal appointments. Eligibility is based on the ability to leave safely and independently, not on occasional supported activities.
